REPLACING A SINGLE TOOTH

One compromised, or missing tooth is easily treated in ONE visit with an “immediate” dental implant. MODERATE SEDATION (NON-IV, ORAL CONSCIOUS)

The most common type of sedation that is administered by our doctors for single site dental implants is a type of moderate sedation that is achieved through the use of pharmacological or non-pharmacologic methods (or a combination). This type of sedation is defined as “an induced controlled state of depressed consciousness."

 

In  this state, patients maintain the ability to breathe independently and respond appropriately to physical stimulation and verbal commands. Enteral Conscious Sedation is administered by taking pills that produce feelings of relaxation and drowsiness. Some degree of amnesia is common.

 

LIGHT SEDATION (NON-IV, NO2)

Light sedation is achieved through the use of a gas called nitrous oxide, also known as laughing gas. The gas is mixed with oxygen and is inhaled through a small mask placed over the nose. Inhalation Conscious Sedation: “Nitrous Oxide/Oxygen Sedation”, which is also known as “laughing gas”, is the most frequently used sedation method used in dentistry. All bodily functions remain normal and the person is able to breathe on their own. The patient will often fall asleep and may experience some level of amnesia.

The most common type of sedation that is administered by our doctors for full arch & full mouth reconstructions is a type of deep (IV) sedation that is achieved through the use of an IV. This type of sedation is defined as “the inability to respond purposefully to either physical stimulation or to verbal command but the patient retains the ability to independently and continuously maintain an airway”.

 

A sedative is directly administered into the blood stream producing a profound state of relaxation to a point where the patient is completely unaware of the treatment. At our office, Board Certified Anesthesiologists administer this type of sedation. Patients receiving this type of sedation must have an adult drive them to and from their dental appointment.

By taking a small pill an hour before your visit, you will be relaxed by the time you arrive. Since you'll be too sedated to drive, you must be accompanied to and from your appointment by a friend or family member. You will be seated in a comfortable chair and covered with a warm blanket. Depending on your needs, we will administer more sedation. Before starting your treatment, we will make sure you are completely comfortable.

At our office, our dentists are trained and certified to administer class I and II sedation while our Anesthesiologists are trained and board certified to administer class III and IV sedation.
 
Class I allows the dentist to administer nitrous oxide. Class II Conscious Sedation is defined as “an induced controlled state of depressed consciousness.” This is the type of sedation being offered at virtually all the dental offices that advertise sedation. Class III Deep Conscious Sedation is defined as “the inability to respond purposefully to either physical stimulation or to verbal command but the patient retains the ability to independently and continuously maintain an airway”.  Class IV Sedation is general anesthesia. At Dentiq (den-teek) Dentistry we offer light to deep sedation including general anesthesia when necessary.

THE DIAGNOSTIC PHASE: ALL ON 4™ (FOUR) DENTAL IMPLANTS

Many patients who have compromised teeth or that wear dentures want to know if they are indeed a candidate for the All-on-4 procedure. The good news is that most people do meet the requirements to efficiently, effectively and safely perform the All On 4 procedure — in fact, 96% of our patients are candidates.  In  order to definitively determine who is an appropriate candidate for this  procedure, (or a treatment alternative) we utilize various diagnostic technologies including 3D imaging and pressure mapping.

 

At the first visit, hi-resolution photos, digital  x-rays, a 3D CAT scan, pressure mapping and a thorough clinical evaluation will take place  followed by expert explanation of the case diagnosis and treatment  options inclusive of the "All On 4" and alternative treatments which can range from single-site implants to replace each missing tooth space, implant retained and implant supported dentures, mini implant solutions and non-implant options.

During this visit, impressions are taken for diagnostic modeling and diagnostic design (which will also be used to make the  replacement teeth), and a clear financial overview is presented. IV SEDATION OR ORAL CONSCIOUS SEDATION: ALL ON 4™ (FOUR) DENTAL IMPLANTS

IV Sedation: All On 4 Dental Implants Is there any pain or discomfort associated with this procedure? Will I feel anything? As with any surgical procedure, there are varying degrees of discomfort. For the "All On 4" patient, the surgical phase is performed under the complete comfort of IV sedation (administered by a board certified Anesthesiologist).   Postoperatively The majority of our "All On 4" dental implant patients comment on how surprised they were with the minimal discomfort they felt after the procedure. There will be minimal swelling, but it's nothing that cannot be controlled with ice and over the counter pain relievers. Patients typically resume a full schedule of activities within 24 to 72 hours. Specific post operative instructions will vary on a case by case basis. Immediate function cases (when we attach the teeth to the implants the same day), patients should maintain a soft / medium diet to allow for the implants to heal properly.  This diet should be followed for a period of 8 to 12 weeks.  In other words, nuts, hard candy, ice and other hard foods are contraindicated during this time period.

While there are significant advantages to the All On 4, there are also inherent risks the patient needs to be aware of.  A primary concern with this technique is the risk of failure. In the event of just one implant failure, the entire restoration may fail.  Load distribution over 4 vs. 6 vs. 8 implants is substantial.

 

Another myth about common problem areas associated with the All On 4 is that it is commonplace that the All On 4 patient will endure jaw atrophy that occurs in the posterior region of the jaw that leads to atrophy and eventually fracture susceptibility. This atrophy poses an increased risk of jaw fracture. Since the all on four technique only places implants in the anterior of the jaw, the bone in the posterior is not stimulated to remain and retain its dentistry; consequently, posterior jaw resorption can occur.

OVERVIEW: ALL ON 4™ (FOUR) DENTAL IMPLANT BRIDGE

A full upper and/or lower arch reconstruction can be comfortably performed under IV sedation in ONE surgical visit with the All-On-4 Dental Implant Procedure developed by the Swiss dental implant company Nobel Biocare.  This procedure uses four implants with the back implants tilted to increase the bone-to-implant contact, which enables the "immediate load" and "immediate function" of the replacement teeth. 

 

For qualified patients, our office honors this treatment type for the life of the patient. At our office, only an Implant Specialist (Oral Surgeon or Periodontist), Prosthodontist and/or Restorative Dentist and a board certified Anesthesiologist performs this type of procedure. THE SURGERY PHASE: ALL ON 4™ (FOUR) DENTAL IMPLANTS

For most cases, this phase involves the extractions, bone grafts (if necessary), bone re-contouring (if necessary) and the surgical placement of the 4 dental implants per upper and / or lower arch. The "All On 4" patient receives implant-secured teeth immediately following surgery — ONE-visit! 

 

RECOVERY & FOLLOW-UP VISITS

Patients typically resume a full schedule of activities within 24 to 72 hours. Generally speaking, follow-up visits are 1, 4 and 12 weeks after surgery.  Specific post operative instructions will vary on a case by case basis. A modified diet is required until patients receive the final teeth, which incorporate concealed titanium reinforced framework. In the mean time, patients can eat anything that can be cut with a fork, and does not involve biting or tearing with your front teeth (e.g. hamburgers, salmon, pasta, cooked vegetables, etc). Hard foods and foods that require tearing such as hard breads, pizza, carrots, corn on the cob, steak, and crunchy and sticky snacks treats such as nuts, hard candy and gum should be avoided until otherwise instructed by the principal dentist.

 

Following surgery, the first 3 follow-up visits are simple thirty minute post-op / follow-up visits to ensure that everything looks, feels and heals as indicated.

THE RESTORATIVE PHASE: OVERVIEW: ALL ON 4™ (FOUR) DENTAL IMPLANTS

The restorative phase starts 16 weeks after the initial surgery when the implants have fully integrated with the jawbone. At "The Prep" visit, the implants are prepared for master impressions so that the final teeth can be made. Depending on the smile design and revision requests, this final fabrication process takes 2 to 4 weeks at which point, the “final” teeth are secured. A standard adjustment visit is scheduled 1 to 2 weeks later to ensure that everything looks, feels and functions exactly as it should.

ALTERNATIVE TREATMENTS TO: THE "ALL ON 4" DENTAL IMPLANT BRIDGE

 

Single-Site Implant Packages ($3,500 to $5,000ea or $44,000 per arch) - Replacing individual teeth with individual implants and crown restorations. This fee includes extraction, bone graft, membrane, guided tissue (if needed), the dental implant, the custom abutment and the final crown restoration. 99.99% of our implants are "single-stage" immediate implants. As a comprehensive dental center, we carry virtually every dental implant that is made with a range of 10 to 25 back-ups per implant.

 

Implant Retained Dentures ($7,700 to $12,500 per arch) - Implant retained dentures are held in place on the implants by clips. These are often referred to as “balls” or “locators”. The denture is much more stable than a conventional denture but they are removable. It does touch the gum and there can be some rocking motion so sore spots are still possible. When using this technique, we wait 3 to 6 months for the implants to fully integrate with the jaw bone before the denture can be held in lace by the clip / ball combination. During this healing period, the patient can still wear a conventional denture and does have the option to immediately secure the denture with additional mini dental implants.

 

Implant Supported Dentures ($17,500 to $21,500 per arch) - Bar overdentures – a fixed metal bar is supported by dental implants. The bar, in turn, supports a removable denture. In this scenario, the denture doesn't actually touch the gums so sore spots are eliminated. This technique requires a 3 to 6 month healing period before the denture can be supported by the bar. Again, during this period, a conventional denture can be worn while the implants integrate with the jaw bone and mini implants can be used as adjunctive therapy to immediately load the denture.

 

Permanent, Fixed, Non-Removable Teeth ($22,000 to $28,500 per arch) - Using the All-On-4 technique, we are able to provide patients with permanent, non removable teeth. This technique achieves the highest aesthetic results and there is usually no waiting for implants to integrate. Patients usually leave with entire arches of permanent teeth on the same day as surgery. The "All On 4" procedure incorporates full arch extractions, avleoplasty (bone re-contouring) bone grafts and the surgical placement of 4 to 6 dental implants that will permanently secure a non-removable bridge comprised of 12 to 14 teeth (per arch). "ALL ON 4" A full upper and/or lower arch reconstruction can be comfortably performed under IV sedation in ONE visit with the All-On-4 Dental Implant Procedure developed by the Swiss Implant maker Nobel Biocare. This procedure uses four to six implants with the back implants tilted to increase the bone-to-implant contact, which enables the "immediate load" and "immediate function" of the replacement teeth. Our office honors this treatment type for the life of the patient. At our office, only an Implant Surgeon, Prosthodontist/ Restorative Specialist & an Anesthesiologist performs this type of procedure. SLActive Implants (+$500ea) - This implant osseointegrates in just 3 to 4 weeks in contrast to traditional implants which require 3 to 4 months to heal.

PAYMENTS: Please understand that we ask for payment at the time of impressions (or) at the time of scheduling (which ever comes first). Depending on the case, we ask for a 5 to 50% deposit, or the full prepayment (b) amount. In the event that you choose not proceed with treatment, and you make an effort in good faith to give adequate notice, the full case prepayment amount (less 5%, and any treatment that has been rendered) will be refunded to you by Dentiq within 30 days.

 

ADJUSTMENTS: 3% adjustment for any amount paid by check (a). 5% adjustment for the full case prepayment (b). Note that prepayment is due at the time of scheduling. FINANCING: Health care financing is available for credit qualified patients through GE Money, Citi Health and Chase Health Advance. Short-term financing terms range from 3.9% to 8.9% interest up to 24 months. Extended-term financing terms range from 9.9% to 14.9% interest up to 60 months.

THE SURGICAL PHASE

A few days to a few weeks later, the surgery takes place. Under non-IV or IV sedation the extractions, bone grafts (if necessary) and implants are administered. Patients can awaken with removable, or implant secured, “initial” teeth to return home with a beautiful smile. Patients resume a full schedule of activities within 24 to 72 hours. Follow-up visits are 1, 4 and 12 weeks after surgery.

SCHEDULING:

You will leave this appointment with a non-removable, implant secured, bridge or individual teeth (4 to 8 hours).  [2] Six weeks (for SLActive cases) to six months (for All On 4 cases) later is "The Final Restoration" visit so that the final teeth with reinforced zirconium or titanium can be placed (4 to 8 hours).

If visiting from another state or country, we ask that you plan your stay to span a 2 day to 2 week duration each visit.  Also, please understand that we perform 5 to 7 full mouth cases a week. LIFETIME GUARANTEE:

When you do your treatment with us, you'll do it once, and it will last a life-time... so we proudly honor the work that we do for the life of the patient.  These are the terms: the patient should complete, and maintain, all clinically necessary treatment; he or she should maintain their regularly scheduled recare visits (3 to 4 x's a yr); and, it should go without saying that the patient should be in good financial standing with our office.  In the event of an accidental injury, or an extraordinary circumstance occurs that compromises the integrity of the treatment, a plan to honor the treatment will be considered on a case-by-case basis.


FINANCIAL AGREEMENT:

All pricing incorporates full arch extractions, avleoplasty (bone re-contouring) bone grafts, ridge augmentation and the surgical placement of all clinically indicated dental implants that will permanently secure individual teeth or a non-removable bridge comprised of 12 to 14 teeth (per arch).

After a comprehensive consultation, you will receive a clear financial overview of the recommended treatment. Our fees reflect final, all-inclusive figures irrespective of additional clinical treatments that may be required for case revisions, refinements or changes. In other words, there will be no financial surprises nor increase in costs what-so-ever. We believe that our comprehensive diagnostic protocol (3D CT Scan, Pressure Mapping, Hi-Res Imaging, Digital X-Rays and Expert Clinical Consultation) provides the diagnostic due diligence to ensure that there will be no clinical surprises either; however, if any additional procedures are required (guided tissue regeneration, plasma rich protein, dental implant replacement or revision, gum grafts, bone grafts, ridge grafts, porcelain restorations, prosthetics, etc) for clinical and / or aesthetic outcomes, they will be performed at no additional cost.

Dentiq Smile Design has pioneered a proprietary process that combines leading imaging, software and refinement technologies to evaluate color, shape, shade, symmetry, size and style with the artistic expressions of design masters (the master ceramist, the principal cosmetic dentist and of course, the patient). QUALIFYING CREDENTIALS

  • Is the "Implant Specialist" a board certified "Periodontist" or "Oral Surgeon" or are they a general dentist who calls him or herself a specialist?

  • Will the IV sedation be administered by a board certified Anesthesiologist or will the same specialist whom is placing the implants also co-administer IV sedation?

  • Is the "Restorative Specialist" a board certified "Prosthodontist" or "Implant Restoration Specialist" or are they the same person who calls him or herself the "Implant Specialist" — A jack of all trades...

II. FMR Smile Design

Dentiq Smile Design has pioneered a proprietary process that combines leading imaging, software and refinement technologies to evaluate color, shape, shade, symmetry, size and style with the artistic expressions of design masters (the master ceramist, the principal cosmetic dentist and of course, the patient). Every detail is incorporated into a digital smile design prior to casting to the temporary veneers (the prototypes). This stage of the protocol requires the master ceramist and the principal cosmetic dentist to submit their design recommendations independently. The patient can be as involved as they would like to be throughout every step of the smile design process. That said, we recommend letting the principal cosmetic dentist and master ceramist develop a design based on measurements, color contrasts, hue, chroma, translucency and applied principles. From this point, patients can typically better articulate design discretions that they would like incorporated into the next prototype for re-evaluation.


Golden Proportion

Beautiful and timeless smiles must be proportionate and remain in harmony with your unique facial features in order to successfully emphasize your true beauty. The roots of this theory can be traced back to hypotheses used during the renaissance, in both art and science. It is a mathematical equation based on the principles of ratio and geometric proportion. This relationship best conveys the philosophy of perfect aesthetic composition, using ratio and dimensional connection. The application of Golden Proportion principles can be applied to the human face. Golden Proportion has been equally applied to the practice of Cosmetic Dentistry, and where consideration is made to essential components of the ‘perfect smile’. These principles of symmetry and balance are the critical ingredients in the development of a ‘beautiful’ face and smile.

 

 

 

 

 

THE DIAGNOSTIC PHASE: ALL ON 4™ (FOUR) DENTAL IMPLANTS 

Many patients who have compromised teeth or that wear dentures want to know if they are indeed a candidate for the All-on-4 procedure. The good news is that most people do meet the requirements to efficiently, effectively and safely perform the All On 4 procedure — in fact, 96% of our patients are candidates.  In  order to definitively determine who is an appropriate candidate for this  procedure, (or a treatment alternative) we utilize various diagnostic technologies including 3D imaging and pressure mapping.    

 

At the first visit, hi-resolution photos, digital  x-rays, a 3D CAT scan, pressure mapping and a thorough clinical evaluation will take place  followed by expert explanation of the case diagnosis and treatment options inclusive of the "All On 4" and alternative treatments which can range from single-site implants to replace each missing tooth space, implant retained and implant supported dentures, mini implant solutions and non-implant options.  During this visit, impressions are taken for diagnostic modeling and diagnostic design (which will also be used to make the  replacement teeth), and a clear financial overview is presented.

 

 

THE SURGERY PHASE: ALL ON 4™ (FOUR) DENTAL IMPLANTS 

For most cases, this phase involves the extractions, bone grafts (if necessary), bone re-contouring and the surgical placement of the 4 dental implants per upper and / or lower arch. The "All On 4" patient receives implant-secured teeth immediately following surgery — ONE-visit!    

 

RECOVERY & FOLLOW-UP VISITS 

Patients typically resume a full schedule of activities within 24 to 72 hours. Generally speaking, follow-up visits are 1, 4 and 12 weeks after surgery.  Specific post operative instructions will vary on a case by case basis. A modified diet is required until patients receive the final teeth, which incorporate concealed titanium reinforced framework OR an all-zirconia substructure. In the mean time, patients can eat anything that can be cut with a fork, and does not involve biting or tearing with your front teeth (e.g. hamburgers, salmon, pasta, cooked vegetables, etc). Hard foods and foods that require tearing such as hard breads, pizza, carrots, corn on the cob, steak, and crunchy and sticky snacks treats such as nuts, hard candy and gum should be avoided until otherwise instructed by the principal dentist.    

 

Following surgery, the first 3 follow-up visits are simple thirty minute post-op / follow-up visits to ensure that everything looks, feels and heals as indicated. For traveling patients, these follow-ups will take place within a 2 day to 2 week span.

 

 

THE RESTORATIVE PHASE: OVERVIEW: ALL ON 4™ (FOUR) DENTAL IMPLANTS

The restorative phase can start 16 weeks after the initial surgery when the implants have fully integrated with the jawbone. At "The Prep" visit, the implants are prepared for master impressions so that the final teeth can be made. Depending on the smile design and revision requests, this final fabrication process takes 2 to 4 weeks at which point, the “final” teeth are secured. A standard adjustment visit is scheduled 1 to 2 weeks later to ensure that everything looks, feels and functions exactly as it should.

 

Week 16: The Prep (4 to 6 hours)

Week 18: The Finals (2 to 4 hours)

Week 20: Adjustments (1 hour)
